Abstract

Objectıve : Posterior urethral valv (PUV) is the most common cause of congenital obstructive uropathy. Although voiding cystourethrogram (VCUG) is accepted as the gold standard for the diagnosis of PUV, its sensitivity varies between 79-100%. The urethral ratio (UO) was used to measure the success of endoscopic valve ablation. The aim of the study is to determine the cut-off value of UO in the diagnosis of PUV.

Materıals and Methods : We retrospectively reviewed patients with or without PUV who underwent cystourethroscopy and VCUG. Two consultant pediatric radiologists calculated UR by dividing the posterior urethral diameter by the anterior urethral diameter. Antenatal hydronephrasis, association of vesicoureteral reflux, blood urea nitrogen, serum creatinine values and glomerular filtration rates were recorded. The chisquare and Mann-Whitney U tests were used for statistical analysis. To determine the cut-off value of UR, receiver-operator curves (ROC) were calculated for sensitivity and spesificity.

Results : The mean UR of 35 patients with PUV (mean age 29.37±43.25 months) and 22 patients without PUV (mean age 15.68±30.74 months) were 2.73±1.08 and 1.61±0.47 respectively. The difference between the mean UR of two groups was significant (p=0.001). ROC analysis showed that the best estimate of UR cut-off value was 1.69

Conclusion: Our results suggested that using the defined cut-off value in our study will be helpful to relieve the inconsistency in interpretation of VCUG for PUV diagnosis.
